Registration for our Fall meeting must be completed by
September 9, 2011.

To Register follow these four easy steps:

1. Download the registration form and print
2. Complete the registration form
3. Make check payable to Illinois Surgical Society
4. Mail the completed registration form and check to:

   Maywood, IL 60153

Registration fees of $200 per person ($150 per guest) must be prepaid.
